This dataset shows the lead testing in school drinking water sampling and results information reported by each NYS public school and Boards of Cooperative Educational Services (BOCES) for Compliance Year 2020.
Public Health Law (Section 110) and New York State (NYS) Department of Health regulation (10 NYCRR 67-4) mandate that NYS public school districts and BOCES test drinking water for lead contamination and report the results to parents, the NYS Department of Health, NY State Education Department, and local health departments. The regulation required school districts and BOCES complete their sampling by the fall of 2016, and test again in 2020, and at least every five years thereafter or at an earlier time as determined by the Commissioner of Health. This dataset shows the results of lead testing in school drinking water reported by each public school district and BOCES for Compliance Year 2020.
The Lead Testing in School Drinking Water regulations may be found here: https://regs.health.ny.gov/book/export/html/56608

The Nursing Home Cost Report (RHCF) is a uniform report completed by New York nursing homes to report income, expenses, assets, liabilities, and statistics to the Department of Health (DOH). Under DOH regulations (Part 86-2.2), nursing homes are required to file financial and statistical data with DOH annually. The data filed is part of the cost report and is received electronically through a secured network. This data is used to develop Medicaid rates, assist in the formulation of reimbursement methodologies, and analyze trends.

The Statewide Planning and Research Cooperative System (SPARCS) Inpatient De-identified File contains discharge level detail on patient characteristics, diagnoses, treatments, services, and charges. This data file contains basic record level detail for the discharge. The de-identified data file does not contain data that is protected health information (PHI) under HIPAA. The health information is not individually identifiable; all data elements considered identifiable have been redacted. For example, the direct identifiers regarding a date have the day and month portion of the date removed.

This dataset represents hospital-reported data for all pediatric patients with a diagnosis of severe sepsis or septic shock seen at New York State Article 28 (acute care) facilities and reported to the New York State Department of Health. Quality measures are calculated using this data for reporting hospital- specific rates and trends. These measures, reported as statewide and hospital-specific rates, reflect several key processes of care that can increase the probability of surviving an episode of sepsis.

This dataset represents hospital-reported data for all adult patients with a diagnosis of severe sepsis or septic shock seen at New York State Article 28 (acute care) facilities and reported to the New York State Department of Health. Quality measures are calculated using this data for reporting hospital-specific rates and trends. The Adult New York State sepsis process of care measures were developed using a National Quality Forum (NQF) measure for guidance: NQF #500 Severe Sepsis and Septic Shock: Management Bundle. These measures, reported as statewide and hospital-specific rates, reflect several key processes of care that can increase the probability of surviving an episode of sepsis. There is one important outcome measure, mortality, included in this dataset for which statewide mortality rates and hospital-specific risk adjusted mortality rates are reported.

The Justice Center for the Protection of People with Special Needs was established in 2013 by the Protection of People with Special Needs Act. The Department adopted regulations to define those adult care facilities that, based on licensed capacity coupled with self-reported quarterly census data, are subject to the conjoined jurisdiction of the New York State Department of Health and the Justice Center for the Protection of People with Special Needs (“Justice Center”). Adult care facilities that are determined to be subject to the conjoined jurisdiction of the New York State Department of Health and the Justice Center are the entities listed in this dataset. For information on how to file a complaint, please refer to the "Notes" section below. For more information about the Justice Center visit https://www.justicecenter.ny.gov/ .

The Behavioral Risk Factor Surveillance System (BRFSS) is an annual statewide telephone surveillance system designed by the Centers for Disease Control and Prevention (CDC). BRFSS monitors modifiable risk behaviors and other factors contributing to the leading causes of morbidity and mortality in the population. New York State's BRFSS sample represents the non-institutionalized adult household population, aged 18 years and older. Although the overall number of respondents in the BRFSS is more than sufficiently large for statistical inference purposes, subgroup analyses can lead to estimates that are unreliable. Interpreting and reporting weighted numbers that are based on a small, unweighted number of respondents can mislead the reader into believing that a given finding is much more precise than it actually is. The BRFSS follows a rule of not reporting or interpreting percentages based upon a denominator of fewer than 50 respondents (unweighted sample). Both a csv and sas data files are available. For more information, check out http://www.health.ny.gov/statistics/brfss/.
