The Statewide Planning and Research Cooperative System (SPARCS) Inpatient De-identified downloadable file contains discharge level detail on patient characteristics, diagnoses, treatments, services and charges. This data file contains basic record level detail for the discharge. The de-identified data file does not contain data that is protected health information (PHI) under HIPAA. The health information is not individually identifiable; all data elements considered identifiable have been redacted. For example, the direct identifiers regarding a date have the day and month portion of the date removed.

This dataset includes information on school reported COVID-19 testing and case positive data beginning on September 13, 2021. Data is collected from private schools on each operational day using the daily school survey form, which school administrators access by logging in to the NYSDOH school survey website. Each school may submit data for the current operational day between the hours of 7am and 5pm. At 5pm, a process runs which aggregates data at the school and district levels. The following morning the data are updated for the previous day and displayed on the School COVID Report Card website.
The primary goal of publishing this dataset is to provide users timely information about disease spread and reporting of positive cases within schools. The data will be updated daily, reflecting data submitted by school administrators the previous day.

This dataset provides the results from collecting and testing adult deer ticks, also known as blacklegged ticks, or by their scientific name Ixodes scapularis. Collection and testing take place across New York State (excluding New York City) from October to December, when adult deer ticks are most commonly seen.
Adult deer ticks are tested in “pools”, or groups of up to ten adult ticks per pool, for the Powassan virus, also known as Deer tick virus. These data should simply be used to educate people that there is a risk of coming in contact with ticks and tick-borne diseases.
These data only provide adult tick minimum infection rates at a precise location and at a point in time. Both measures, tick population density and minimum infection percentages, can vary greatly within a very small area and within a county. These data should not be used to broadly predict disease risk for a county.
Further below on this page you can find links to tick prevention tips, a video on how to safely remove a tick, and more datasets with tick testing results. Interactive charts and maps provide an easier way to view the data.

This dataset includes information at the report date level by individual facilities on the total number of staff, how many staff are partially vaccinated, and how many are fully vaccinated. This information is only collected once a week from hospitals. The title of this dataset was initially the Hospital Electronic Response Data System (HERDS) Hospital Survey: Hospital Staff COVID-19 Vaccinations. The dataset was changed to its current title on 11/4/2021.

This dataset includes information on the number of tests of individuals for COVID-19 infection by zip code performed in New York State beginning March 1, 2020, when the first case of COVID-19 was identified in the state. The primary goal of publishing this dataset is to provide users timely information about local disease spread and reporting of positive cases. The data will be updated weekly, reflecting tests completed by 2:00 pm on the day prior to the date of the update.
Note: On November 14, 2020, only 14 hours of laboratory data was collected and shared. The 2:00 pm cutoff time was implemented, allowing the NYSDOH to enhance data quality reviews. All other published laboratory data represented 24 hours of data collection. Prior to November 14, 2020 data reflected tests completed by 12:00 am (midnight) the day of the update (i.e., all tests reported by the end of the day on the day before the update).
As of 4/4/22, the Department of Health and Human Services (HHS) no longer requires entities conducting COVID testing to report negative or indeterminate antigen test results. This may impact the number and interpretation of total test results reported to the state and also impacts calculation of test percent positivity. Total positives continues to include both PCR and antigen positive test results.

This dataset provides the results from collecting and testing nymph deer ticks, also known as blacklegged ticks, or by their scientific name Ixodes scapularis. Collection and testing take place across New York State (excluding New York City) from May to September, when nymph deer ticks are most commonly seen.
Nymph deer ticks are tested in “pools”, or groups of up to ten adult ticks per pool, for the Powassan virus, also known as Deer tick virus. These data should simply be used to educate people that there is a risk of coming in contact with ticks and tick-borne diseases.
These data only provide nymph tick minimum infection rates at a precise location and at one point in time. These data should not be used to broadly predict disease risk for a county.

Sepsis is defined as a clinical syndrome in which patients have an infection that is accompanied by an extreme systemic response. Sepsis of sufficient severity that the function of major organ systems in the body (such as heart, kidney, brain, and others) is impaired is referred to as “severe sepsis.” Patients with severe sepsis that have continued organ system impairment and/or low blood pressure that does not respond to treatment with adequate fluid replacement are considered to be in “septic shock.” The combination of early detection of sepsis coupled with timely, appropriate interventions can significantly improve the chances of survival for patients with all types of sepsis.
The datasets contain hospital-level treatment measures and hospital-level risk-adjusted mortality rates for adult patients, with a diagnosis of severe sepsis or septic shock seen at New York State Article 28 (acute care) facilities and reported to the New York State Department of Health in 2015 and 2016.
The treatment measures and risk adjusted mortality rates are presented by hospital. Only hospital level measure data for which there were at least ten cases in the denominator is reported. Statewide measures are calculated using all hospitals, regardless of the number of sepsis cases treated.

This dataset contains Potentially Avoidable Antibiotic Prescribing observed and risk-adjusted rates for adult Medicaid enrollees by provider county beginning in 2010.
Potentially Avoidable Antibiotic Prescriptions are antibiotic prescriptions filled for the treatment of acute respiratory infections for which antibiotics are not indicated, contributing to bacterial drug resistance. Index visits for acute respiratory infections and corresponding prescription fills were identified through the use of previously published methods.
The rates were calculated using Medicaid outpatient claims and encounters, and prescription drug data.
The observed and risk adjusted rates are presented by provider county (including a statewide total).

This dataset represents hospital-reported data for all adult patients with a diagnosis of severe sepsis or septic shock seen at New York State Article 28 (acute care) facilities and reported to the New York State Department of Health. Quality measures are calculated using this data for reporting hospital-specific rates and trends. The Adult New York State sepsis process of care measures were developed using a National Quality Forum (NQF) measure for guidance: NQF #500 Severe Sepsis and Septic Shock: Management Bundle. These measures, reported as statewide and hospital-specific rates, reflect several key processes of care that can increase the probability of surviving an episode of sepsis. There is one important outcome measure, mortality, included in this dataset for which statewide mortality rates and hospital-specific risk adjusted mortality rates are reported.

The Nursing Home Cost Report (RHCF) is a uniform report completed by New York nursing homes to report income, expenses, assets, liabilities, and statistics to the Department of Health (DOH). Under DOH regulations (Part 86-2.2), nursing homes are required to file financial and statistical data with DOH annually. The data filed is part of the cost report and is received electronically through a secured network. This data is used to develop Medicaid rates, assist in the formulation of reimbursement methodologies, and analyze trends.
